Using a Power of Attorney to transfer real estate

I have a Power of Attorney for one of my relatives, and I need to
transfer real estate from this person's name into mine using a Warranty
Deed. I know the POA gives me the authority to do this, but I am
unsure whether I need to sign the relative's name or mine and whether I
should sign the suffix "POA".
Thank you!

Sign your name and append "for" + the person's name. You don't go into
details, but to me, this transaction seems bizarre.
