Here is one of several Web sites where you can get all your questions
answered plus also get the LLC done. Yes, you can use your residence as
your company address barring local ordinances forbidding same.
http://www.inc-llc.com/
I have no idea what an EAD card is so I won't comment further.

The creation and regulation of LLC's are performed at the state level. Check
the web pages for your state government, usually under the Secretary of
State, or other web site which discusses the forming of business entities.
The cost will vary from about $50 to several hundred dollars depending on
the state. In some states you can do this completely on-line. Before you
start the process, you will need to come up with a unique business name
(unique to that state). In some states, you will need to pay extra to do a
name search and to reserve a name. It would help if you have several
different names you are willing to use in case the one you want is already
taken.
You usually do not need a lawyer or accountant, especially if you are a sole
proprietor (no other owners). You will simply file your business taxes using
1040 Schedule C on your personal tax return. You must keep accurate records
of you revenues and expenses and be able to prove they are business related.
You may use your home address as the address of your business, and you may
be able to deduct part of the cost of your home that is used exclusively for
business purposes (but see IRS regs for details).
You will need a social security number, and if you have employees other than
yourself you should have an IRS EIN (business tax id number) . Search the
IRS website for how to get one.
