To file for a name change for a minor, my county clerk of circuit courts
asked me to present the following forms:
Petition for change of name
Notice of hearing
Order for hearing
Order for change of name
I saw samples of these forms and the first one is fairly straightforward.
But I don't seem to have enough information for the other forms. For
example, in the "notice of hearing" I am supposed to specify the date in
which the hearing will be held, which at the time of filing I would not
know. I presume I will leave this information blank?
