adding a middle name

I got all the forms to file for a "name change." This has to do with adding
a middle name for my daugther.
Is it even necessary?
I know that one can use a middle initial and even a middle name without
necessarily having to go through a name change. But at one point does it
become an obligation/necessity?

accomplished by simply using a new name in all aspects of your personal,

Only if she is under age. Once someone turns 18 they can simply start calling
themselves anything they want. Otherwise a woman would have to get a court
order when she starts using her husband's last name after they get married.

When you're below legal age.
