Quitclaim Deed Form Questions

I recently prepared a revocable living trust, and now I am transferring my
assets into it. I am the trustee of the revocable living trust.
Regarding the transfer of my condo to the trust, I need to complete the
Quitclaim Deed form for the state where I live and take it to my County
Recorder. In preparing the form, which I just downloaded from the Internet
(it is a Word document that can be edited), I have a few "simple" questions
that I would appreciate some assistance.
1) In the GRANTEE portion, the form has a blank space, followed by "an
individual" and a place to put an "X" next to married or unmarried. Should
I just blank all this out and put the name of the trust, ie, "my name" ,
trustee of the "my name" Revocable Living Trust, dated "date of trust"?
Just how should this read?
2) After that section, the form has
"WITNESSETH, That the Grantor, for good consideration and for the sum of
___________ Dollars ($____) in hand paid"
What do I put in $0, or something else?
3)Further down on the form, it has "THE TOTAL DOLLAR VALUE OF THE
CONSIDERATION paid for the property described herein is $__."
What do I put in here - 0?

one dollar for the consideration (can I assume I put "$1" in both sections
of the form? And why $1 instead of $0?), yourself as trustee for grantee
(can I assume you mean here "Les Doe, trustee of the Les Doe Revocable
Living Trust, dated December 1, 2003". Also is it OK to blank out "married"
or "unmarried" or should I just X the one that is applicable?
