I want a restraining order against my dog

Last week my dog bit me. I moved out of my house and left my dog. I have since moved into an apartment, and have not gone back ot my home knowing the dog is there. I am afraid the dog might find my apartment and bite me again. I want to get a restraining order against my dog so he dont come near me anymore. How can I get one? Marie
